Clinical Pharmacist

About the role

Pharma Life is looking for a qualified, motivated, and patient‑focused Clinical Pharmacist to join its healthcare team in Kathmandu. The role involves delivering clinical pharmacy services, optimizing medication therapy, and collaborating with doctors, nurses, and other professionals to improve patient outcomes.

Key responsibilities

• Review and evaluate prescriptions for appropriate drug selection, dosage, route, frequency, and duration.
• Monitor patients' medication therapy, identify drug‑related problems, and assess drug‑drug interactions, contraindications, allergies, and adverse reactions.
• Participate in clinical/ward rounds and provide pharmaceutical care recommendations.
• Provide medication counselling and education to patients and caregivers.
• Supply evidence‑based drug information to healthcare professionals.
• Document and report adverse drug reactions and medication‑related incidents.
• Support antimicrobial stewardship and promote rational use of medicines.
• Conduct medication reconciliation during admission, transfer, and discharge.
• Assist in prescription audits, medication‑use evaluations, and development of hospital formularies and treatment guidelines.
• Maintain accurate clinical pharmacy records and contribute to medication safety and quality‑improvement initiatives.
• Participate in clinical research, audits, training, and professional development activities.

Required profile

• Pharmacy degree with a valid license to practice.
• Minimum of 2 years of clinical pharmacy experience.
• Strong knowledge of medication therapy management and pharmacovigilance.
• Excellent communication and teamwork skills.
• Patient‑centered attitude and ability to work in a multidisciplinary environment.
