Home Health and Telehealth Nurse

About the role

We are seeking a compassionate and skilled Home Visit & Telehealth Support Nurse to deliver quality nursing care through in‑home visits and digital consultations. The role blends direct patient care, diagnostic sample collection, chronic disease monitoring, and telephonic support to ensure safe, patient‑centered healthcare.

Key responsibilities

• Conduct home visits for patient assessments, nursing care, and follow‑up consultations.
• Collect, label, and safely transport diagnostic samples, including venipuncture.
• Monitor elderly patients and individuals with chronic conditions such as hypertension, diabetes, and post‑dialysis care.
• Handle patient calls, inquiries, and provide telehealth support, escalating to physicians when needed.
• Maintain accurate patient records, medical documentation, and adhere to infection control and confidentiality protocols.

Required profile

• Diploma in Nursing (GNM) or BSc Nursing from a recognized institution.
• Valid nursing license with current CPR/BLS certification.
• Minimum 2 years of clinical nursing experience, preferably in home healthcare, phlebotomy, elderly care, or chronic disease management.
• Valid driving license and reliable transportation for home visits.
